About The Position

The Care Coordinator position at the City of Tucson's Community Safety, Health & Wellness (CSHW) program within the City Manager's office provides follow-up for non-emergency 911 calls and referrals from City of Tucson departments to assist residents in accessing the appropriate social service agencies within the community, reducing barriers to obtaining necessary support, and preventing future crises. The Care Coordinator acts as a social worker/case manager to facilitate individuals or households in getting and staying connected to reliable community-based resources. Work is performed under the supervision of the Community Safety, Health & Wellness Resource Manager. This position does not supervise.

Requirements

  • Bachelor's degree.
  • Two (2) years of directly related experience.
  • Any combination of relevant education and experience may be substituted on a year-for-year basis.
  • Valid and Unrestricted Class D - Driver License - Arizona Motor Vehicle Department.

Nice To Haves

  • Fluency in Spanish, Arabic or French.
  • Peer Support Specialist or Behavioral Health Technician Certification.

Responsibilities

  • Provides case management through identifying and coordinating appropriate community resources to meet client's needs, including social services and community-based support.
  • Applies case management and crisis intervention best practices as alternative solutions to the 911 system to address and prevent current or future crises.
  • Manages caseloads of clients with various and dynamic needs such as aging, homelessness, behavioral health, and substance misuse.
  • Follows-up with clients until needs are successfully met and the individual or household is stable.
  • Responds and processes referrals from various City of Tucson Departments and the public including City residents, businesses, churches and social service agencies.
  • Processes client intakes by meeting with clients on the phone or in their home to gather basic information including perceived needs, income, existing case worker information and current social, mental, physical or economic supportive services.
  • Collects case data and maintains accurate records, ensuring operational efficiency by entering data on a timely, complete, and accurate basis.
  • Maintains up-to-date records of all referrals, intakes and case management activities in the program's digital database and other platforms as needed.
  • Collaborates with CSHW Program management, City of Tucson community partners, and other social service providers in coordination of and service to the Community Safety, Health & Wellness program operations.
  • Participates in neighborhood meetings, public meetings, public forums and team meetings as needed.
  • Attends trainings and participates in continuous quality improvement of the program relating to program operating procedures, behavioral health, social services including housing, job development skills, and other City of Tucson staff requirements.
  • Performs all other duties and tasks as assigned.
