About The Position

The Care Coordinator facilitates the quality and improvement of a person's experience of care, which may include admission, discharge, outreach, engagement, recovery, transition and case management.

Requirements

  • Excellent computer skills, including Microsoft Office Suite.
  • Demonstrated ability to effectively communicate both verbally and in writing.
  • Excellent interpersonal skills.
  • Excellent customer service skills.
  • Demonstrated organizational skills and high degree of accuracy.
  • Works cooperatively in a team environment.
  • Ability to provide courteous assistance in a concise, informative manner, keeping a positive tone and appropriate non-verbal language.
  • Bachelor's Degree in a behavioral health field and a minimum of one (1) year of behavioral health experience; or Bachelor's Degree in any field and a minimum of two (2) years of behavioral health experience; or High School Diploma or equivalent and a minimum of four (4) years of behavioral health experience.
  • Must be able to pass de-escalation training, as defined by state requirements.
  • Ability to maintain a high level of confidentiality (HIPAA guidelines and regulations) and professionalism.
  • Must be able to pass a seven-year background check.
  • Must be able to pass substance abuse testing and/or tuberculous testing (if applicable).
  • Must be able to pass education verification check.
  • Must be able to obtain designated fingerprinting clearance in applicable states.
  • Must be able to pass three-year motor vehicle report and meet minimum eligibility requirements criteria of insurance carrier (if driving is required for this position).

Responsibilities

  • Provides coordination of care of services and resources available internally as well as throughout the community.
  • Acts as an advocate for the needs and rights of individuals.
  • Coordinates or facilitates multidisciplinary team meetings to ensure services are provided in a timely manner.
  • Assists individuals in daily living activities and establishing and maintaining linkages that provide education, support and independence in the community.
  • Partners with other provider agencies in the coordination of care to help participants achieve wellness.
  • Documents relevant and required information and services assuring coordination of care as a tool for communication with other team members.
  • Understands and adheres to RI International compliance standards as they appear in the Code of Conduct and Conflict of Interest Policies.
  • Performs other related duties as assigned.
