About The Position
Sentara Health Plans is hiring a Care Coordinator in Norfolk, VA Beach, Suffolk, Chesapeake, Portsmouth, VA and surrounding areas! This is a full-time, permanent position requiring 40 hours of work per week. The standard working hours are from 8am to 5pm EST, Monday to Friday. The position requires both in-person face-to-face assessments and remote telephonic assessments of our members. Candidates must reside in Norfolk, VA Beach, Suffolk, Chesapeake, Portsmouth, VA and surrounding areas.
