About The Position

The Care Coordinator plays a critical role in supporting patients, providers, and operational workflows across Bastion Health. This position blends patient outreach, provider coordination, clinical operations, and logistics management. The ideal candidate is organized, proactive, comfortable speaking with patients, and excited to help people navigate their healthcare journey. This role is hybrid, with approximately 2–3 days per week in our Farmington, CT office and the remainder remote.

Requirements

  • BA or BS required in a relevant field
  • 1–3 years of experience in healthcare administration, care coordination, or a related role
  • Health coaching license a must
  • Strong communication skills and comfort speaking with patients by phone
  • Highly organized with strong attention to detail
  • Ability to manage multiple operational workflows simultaneously
  • Comfortable working with digital health platforms and EMR systems
  • Ability to work in a hybrid environment (2–3 days in office)

Nice To Haves

  • Experience working with EMR systems (Athena preferred)
  • Experience in telehealth or digital healthcare environments
  • Familiarity with logistics, shipping platforms, or inventory management

Responsibilities

  • Conduct patient outreach to schedule appointments and assist with onboarding
  • Create personalized patient communications via phone, text, and email
  • Answer patient questions and provide guidance on services and care plans
  • Coordinate communication between patients and providers
  • Encourage patient adherence to care plans and follow-up appointments
  • Obtain patient demographic information, identification, and insurance documentation
  • Conduct referral outreach calls when needed
  • Assist patients in resolving simple technical issues related to telehealth visits
  • Maintain and monitor provider availability and schedules
  • Assist with onboarding and training of new providers
  • Provide refresher training or operational guidance to existing providers
  • Monitor visit documentation and ensure visit note compliance
  • Escalate potential standard-of-care or compliance issues for review
  • Check in patients for visits using Bastion's dashboard and Athena EMR
  • Support providers by submitting lab and medication orders when appropriate
  • Coordinate claims submission and fulfillment processes
  • Ensure smooth patient visit flow and documentation completion
  • Assemble at-home testing kits
  • Manage inventory of testing supplies and materials
  • Oversee kit shipment and fulfillment
  • Coordinate with lab partners, including IHD and Fellow Labs
  • Manage shipping and tracking using ShipStation
