About The Position

The Care Coordinator will work as a conduit between individuals served, clinical staff, and community providers to ensure services are provided, documented, and communicated between internal and external providers. The Care Coordinator is responsible to close the referral loop and follow-up with individuals to determine if they were able to access the services to which they were referred. Releases of information will be obtained to assist with communication between PVC and outside entities. Must be familiar with and/or willing to be training in trauma informed care. This includes understanding, recognizing, and responding to effects of all types of trauma.

Requirements

  • Considerable knowledge of principles, methods, procedures, and practices of community mental health services related to mental health crisis intervention is necessary.
  • Ability to make effective assessments of and interventions in situations involving PVC services and consumers is needed.
  • Must be able to bend, stoop, kneel, crouch, squat, lift 25 pounds, sit for an extended period, and reach overhead.
  • High School Diploma / GED required.
  • A valid Texas driver’s license and good driving record is required along with a copy of a valid auto insurance card.

Nice To Haves

  • Knowledge of rural community organizations, functions and relationships is also a plus for this position.
  • It is also necessary to maintain a flexible work schedule.
  • Considerable use of electronic health records and computer programs will be required.
  • Bachelor’s degree in Human Services field preferred.
  • Prefer direct experience with community mental health, primary care, or in a human services field.
