The Care Coordinator actively engages with members/recipients through comprehensive assessment, care planning, health promotion, and comprehensive transitional care. The Care Coordinator provides time-limited community-based Care Coordination for members not eligible for, or have opted out of Tailored Care Management. The employee must be able to work effectively with internal and external stakeholders to fulfill duties and responsibilities. Travel is an essential function of this position.
