About The Position

- The Care Coordinator at LIFESTREAM BEHAVIORAL CENTER is responsible for managing and coordinating comprehensive care plans for clients to ensure they receive the necessary behavioral health services. This role involves working closely with clients, families, and healthcare providers to facilitate access to appropriate resources and support systems, ultimately aiming to improve clients' overall well-being and quality of life.

Requirements

  • Bachelor's degree in Social Work, Psychology, Nursing, or a related field from an accredited institution.
  • Minimum of 2 years of experience in a healthcare or behavioral health setting.
  • Proven track record of coordinating care plans and working collaboratively with multidisciplinary teams.
  • Strong understanding of mental health and behavioral health concepts.
  • Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals.
  • Ability to assess patient needs and develop individualized care plans.
  • Proficiency in using electronic health records and other healthcare management software.
  • Strong organizational skills with the ability to manage multiple cases and prioritize tasks effectively.
  • Demonstrated ability to work independently and as part of a team in a fast-paced environment.
  • Knowledge of community resources and ability to connect patients with appropriate services and support.

  • Experience in case management and patient advocacy is highly desirable.

Responsibilities

  • Develop, implement, and monitor individualized care plans for clients, ensuring alignment with their specific needs and treatment goals.
  • Collaborate with healthcare professionals, social workers, and other stakeholders to coordinate services and resources for clients.
  • Conduct regular assessments and evaluations of clients' progress and adjust care plans as necessary to achieve optimal outcomes.
  • Serve as the primary point of contact for clients and their families, providing guidance, support, and education regarding available services and resources.
  • Facilitate communication between clients, families, and healthcare providers to ensure a cohesive approach to care.
  • Maintain accurate and up-to-date records of client interactions, care plans, and progress reports in compliance with organizational and regulatory standards.
  • Advocate for clients' needs and preferences, ensuring they have access to necessary services and support.
  • Participate in multidisciplinary team meetings to discuss client cases and contribute to the development of comprehensive care strategies.
  • Stay informed about the latest developments in behavioral health services and incorporate best practices into care coordination efforts.
