Jefferson Center is seeking a Care Coordinator to join our recovery oriented, Recovery Clinic. The Recovery Clinic is an interdisciplinary treatment approach rooted team collaboration, data, and measurement, where each member contributes their expertise. The latest team-based clinical practices and tools are employed to help those in need get better fast and maintain their health and wellbeing long-term. We seek to offer our community access to the best behavioral health, substance use, and wellness services starting on the same day they are requested, without exception. We will achieve this while simultaneously creating a work environment where our team has the autonomy, support, and tools to be wildly effective, satisfied, and engaged at work. Recovery Clinic: Substance Use and Co-occurring Outpatient Care provides immediate, non-crisis support and therapeutic services for adults 18 and older and their significant caregivers/families. This includes Indigent clients, clients with Medicaid coverage or self-pay for therapeutic trauma-informed services. The SUD and Co-occurring Outpatient team provides wraparound clinical services from care coordination, Initial and concurrent assessments, individual and/or family therapy, case management and medication management services. Walk in, with flexibility to provide some scheduled appointments Team of equals – regular touch points and consultation Provide excellent customer service Generate new ways to help people get better faster Use tech to analyze trends Use tech to outreach clients and engage Participate in team operation workgroups to advocate for program development opportunities

Requirements

  • Bachelor’s degree in social work, nursing, human services, counseling, psychology, or another related field required.
  • Ability and willingness to work in face-to face client settings, out in the community, or via video telehealth
  • Experience working with vulnerable populations of diverse cultures and ethnicities
  • Highly reliable, flexible, organized, and self-motivating
  • Skills, experience, or vested interest in integrated health care approaches
  • Comfort with or willingness to quickly learn various technologies

Nice To Haves

  • Bilingual (English/Spanish) preferred
  • Knowledge of and experience with community-based services and supports in the area are highly valued

Responsibilities

  • Engage consumers, appropriate family members, and significant others as requested and consented to by client, in the strength-based and trauma-informed treatment process and goal setting
  • Assist and support clients in gaining access to needed medical, social, educational, and other services
  • Provide care coordination and case management
  • Administer evidence-based measurement tools regularly and consume out data to drive clinical collaboration discussions and consequently, a client’s care planning
  • Identify risk factors such as lethality for suicidal, homicidal, and/or grave disability and coordinate with emergency services as necessary
  • Participate in evidence-based practices and other agency initiatives for mental health and co-occurring substance use services
  • Exhibit enthusiasm, courtesy, adaptability, and spirit of cooperation and teamwork in the work environment
  • Effectively respond to client needs and problems; initiative and maintain positive interactions; timely response to messages and other requests
  • Utilize data and new operational practices to better adapt caseload complexity.
  • Provide case management services to clients (and/or families)
  • Promote protective factors and support with accessing basic needs
  • Coordination for emergency services when clinically indicated or medically necessary; risk/lethality assessment
  • Maintain knowledge of and facilitate connections with Jefferson Center and community resources; develop and maintain relationships with entities to assist in a seamless referral process
  • Possess a working understanding of the concepts of Healthcare Equity, Health Disparities and Cultural Relevance and of their impact on service planning, service coordination and service provision
  • Possess awareness of the relationship between social indicators (such as immigration history, racism, access barriers, poverty, residency status, etc.) and biopsychosocial functioning
  • Consistently meets the expectations of the job
