Care Coordinator Community-Based | Children & Families Do you believe strong relationships change lives? We’re looking for a Care Coordinator who thrives in relationship-based work and wants to make a meaningful difference for children, adolescents, and their families. In this role, you’ll walk alongside clients and caregivers, helping them navigate services, build on strengths, and create sustainable support systems that truly work in their daily lives.

Requirements

  • High school diploma and Bachelor’s degree or other approved work/personal experience in working with children
  • Two years working with children, preferably experience including children with severe emotional disturbance and their families
  • Strong understanding of child and adolescent development, including typical and atypical behaviors.
  • Ability to set and maintain healthy boundaries while building trusting relationships.
  • Knowledge of safety considerations and appropriate interventions.
  • Strong organization, communication, and time-management skills.
  • Comfort working independently while staying connected to a team.
  • Valid Kansas Driver’s License, reliable transportation, and proof of insurance.
  • Must be able to pass Kansas Bureau of Investigation Screen, Central Registry Screen, and DCF (Kansas Dept. for Children & Families, formerly SRS) background check

Responsibilities

  • Provide care coordination services to children/adolescents and their families in community-based settings.
  • Build ongoing, supportive relationships with clients and caregivers.
  • Facilitate initial treatment planning meetings and ongoing reviews every 90 days.
  • Assess family strengths, needs, and community connections.
  • Support client autonomy by helping youth and families build skills, resilience, and community integration.
  • Provide coaching and guidance to families to increase understanding of their child’s needs and service options.
  • Coordinate with therapists to update goals, objectives, and services as needed.
  • Assist with crisis intervention in collaboration with other care team members.
  • Serve as a liaison between families, Prairie View staff, and community agencies.
  • Build relationships with schools, service providers, and community partners to create natural support networks.
  • Participate in treatment team meetings and wrap-around planning.
  • Communicate regularly with internal staff to ensure coordinated, high-quality care.
  • Maintain timely and accurate clinical documentation and records.
  • Track time and services in accordance with agency and state standards.
  • Update Plans of Care and waiver documentation as required.
  • Coordinate with Utilization Review staff to ensure service authorization is in place.
  • Manage a caseload while prioritizing client and agency needs.
  • Meet documentation timelines and encounter expectations.
  • Participate in supervision, training, and required state-approved programs.
  • Complete ongoing education related to mental health, wrap-around services, and best practices.
  • Engage in workshops, seminars, and in-service trainings to support professional growth.
