Under the supervision of the Program Director, the Care Coordinator is responsible for coordinating care for all ROWI clients. This includes individualized service coordination such as orientation, transportation, doctor visits, and scheduling. The role ensures high standards of program implementation and client care, including supervision during groups, facilitating activities, conducting client orientations and assessments, and maintaining compliance with Joint Commission Standards. The Care Coordinator also handles documentation, coordinates and provides client transportation, assists with facility cleanliness, and manages daily client lunches. They are expected to serve as a positive role model, maintain confidentiality, adhere to dress code, complete tasks efficiently, and attend meetings. The position requires the ability to maintain clear boundaries with clients and staff, manage clients needing direct supervision, and engage in professional interactions and communication.
