About The Position

The Care Coordinator uses a motivational, stage-matched approach to coordinate care for clients throughout all phases of care to collect, analyze, and communicate information on the status of client heath and current needs. This is a full-time position working Monday-Friday, 8:00AM - 5:00PM. What you will be doing: Work collaboratively with clints/family, focused on supporting client's movement towards recovery/resiliency goals. Communicate actively with members of the integrated team, including psychiatrists, primary care physicians, specialist, regarding all areas of client's wellness, including client's adherence to medications, side effects, and efficacy of medication. Seek and include input from Integrated Health Team providers. Assist in accessing public services, including financial, housing, emergency assistance. Maintain contact with clints who are hospitalized, participating in discharge planning and communicating with all involved providers in developing aftercare plans. Assist clients with developing personal support systems, including but not to limit to work with family members, legal guardians, recovery communities, etc. Provide individual assistance to clients in accessing needed services including medical and other identified health needs. Follow up with clients regarding appointments, completion of forms, returning forms or receipts and other similar activities. Contact clients who have missed appointments in order to re-engage them and per mote recovery efforts. Meet service provision expectations per client needs and center guidelines. Complete documentation and paperwork within the required timeframe. Complete other duties as assigned. Requirements

  • Bachelor's degree in Counseling, Psychology, Social Work, or other human services field.
  • High School diploma: If no degree has been obtained, candidate must have at least 4 years of experience or combination of higher education, all experience must be with individuals with psychiatric, substance use and or developmental disabilities.
  • Someone with knowledge of community resources and social service agencies, advocacy, crisis de-escalation and intervention techniques, and an understanding of trauma informed, recovery-oriented, and evidence-based practices.
  • Demonstrated experience with diverse populations receiving behavioral health treatment, including delivery of treatment to individuals with severe mental illness, substance use disorders, neurodiverse conditions, and psychosocial needs (poverty, homelessness, justice involvement, etc.)
  • Strong organizational skills and the ability to work with a multidisciplinary team in a demanding and dynamic environment.
  • Effective time management skills, strong communication, collaboration, and relationship skills to effectively engage with consumers and their families and community partners, and the ability to deal with difficult or demanding situations in a friendly and constructive manner.
  • Excellent problem-solving skills with the ability to analyze alternative solutions and work to resolve issues in a timely manner.
  • A valid driver's license with acceptable MVR and agency established minimum automobile coverage.
