About The Position

A Care Coordinator at Families Together of Orange County, plays a crucial role in ensuring a seamless healthcare journey for our patients. Collaborating with diverse healthcare providers, the Care Coordinator will advocate for patients, update documents accurately, and schedule appointments to guarantee timely and appropriate medical care. Handling specific cases for community access programs, you'll address barriers such as access to care and transportation, ensuring elderly or disabled clients receive the healthcare they need. Your organizational prowess, coupled with exceptional communication skills, will contribute to creating and implementing care plans for multiple patients, fostering a compassionate and understanding environment within our healthcare services.

Requirements

  • Bachelor’s degree in social work, nursing, health, human services or closely related field;
  • Knowledge and experience in working with diverse communities in the Orange County, CA area.
  • Understanding of Medicaid Waiver systems and community resources.
  • Bilingual: Spanish (Required)
  • Minimum 2 years of related experience.
  • Minimum 2 years of experience in the human services field. Additional experience may be substituted for the academic requirements identified above.
  • Must excel in multitasking within a high-paced environment.
  • Strong computer skills, acute attention to detail, confident and professional communication.
  • Responsiveness to the needs of both internal/external stakeholders in a professional and personable manner are expected.

  • Knowledgeable in general EMR/EHR systems is preferred.

Responsibilities

  • Patient Advocacy: Serve as a dedicated advocate for patients, liaising between them and healthcare providers to address their needs and concerns.
  • Communication Facilitation: Effectively communicate with diverse healthcare professionals, ensuring accurate and timely updates of patient documents and facilitating seamless appointment scheduling within FTOC and other specialist offices.
  • Community Access Programs: Handle assigned cases within community access programs, managing healthcare communications on behalf of elderly or disabled clients, and addressing barriers such as access to care and transportation as needed.
  • Care Plan Development: Utilize strong organizational skills to systematize care plans for multiple patients, coordinating with healthcare workers with varied schedules.
  • Verbal and Written Communication: Demonstrate highly developed verbal and written communication skills to expedite patient understanding of health needs while maintaining respect and confidentiality.
  • Barrier Resolution: Work proactively to identify and overcome barriers that could prevent patients from accessing healthcare services, including payment plans, transportation, and other logistical challenges.
  • Compassionate Guidance: Guide patients and healthcare teams with compassion, ensuring a complete understanding of healthcare procedures and practices.
  • Collaborative Approach: Collaborate with interdisciplinary teams to foster a compassionate and understanding environment within our healthcare services.
  • Undertakes other tasks for which training has been provided, as assigned.
