About The Position
The Council on Recovery is a well-respected not-for-profit organization dedicated to maintaining the health, productivity, and safety of the community by providing essential services and information to individuals affected by alcohol, drugs, and related issues. As a Care Coordinator, you will play a pivotal role in managing and coordinating client care through various support channels, including telephonic communication, chats, emails, and direct client interactions for walk-ins. This position is designed for individuals who are passionate about helping others and are committed to making a difference in the lives of those struggling with substance use disorders. In this role, you will spend significant time engaging with clients, assessing their needs, and providing them with the necessary information and resources. You will be responsible for crisis intervention services and facilitating warm hand-offs to both internal and external programs, ensuring that clients receive the comprehensive support they need. Additionally, you will triage client needs and ensure timely follow-up for required services, making sure that no client falls through the cracks during their recovery journey.
