About The Position
The Care Coordinator is a key member of the Behavioral Health Team and serves as an advocate for patients by coordinating care to minimize the wait for services, effectively assess needs of patients, and increase engagement in therapy and integrated services. They directly support evolvedMD's Behavioral Health Managers (BHMs) by promoting effective engagement of patients referred for external support services.
