The Care Coordinator is responsible for creating extraordinary client and family experiences and ultimately manages 'best in home care' care plans. Build, manage, and adjust caregiver schedules to meet changing client needs. Coordinate last-minute shift coverage efficiently and proactively. Communicate scheduling updates quickly and clearly to caregivers and clients. Develops and maintains the client care plan, creating extraordinary client experiences and communicating with clients, families, care partners and referral sources. Act as the primary contact for caregivers—checking in, resolving issues, and boosting morale. Plays an integral role in interfacing with clients and families to ensure safe, appropriate and meaningful care is being provided. Serves as a mentor to caregivers and supports their care delivery. Occasionally perform in-person assessments and support care plan updates. Support general office operations, including answering phones and handling inquiries. Maintains a calm, positive demeanor at all times and is able to effectively multitask in a high functioning office environment.
