About The Position
In this role, the Care Coordinator will be responsible for utilizing an integrated case management model that includes assessment, planning, implementation, and monitoring. The Care Coordinator is responsible for the management of the member's whole health and will provide the services and coordination that are needed/requested. The Care Coordinator will be responsible for encountering services within the Electronic Health Record.
