The HouseCalls Clinical Support Team (HCCST) supports Advanced Practice Clinicians in the HouseCalls and Complex Care Management businesses by providing telephonic consultation to plan members and their healthcare providers in a call center environment. This team includes nurse care managers, and social workers. This role is 100%25 Remote but does require a work schedule of 9am-530pm EST or CST, Monday-Friday No On-Call or Holidays The Clinical Care Coordinator- Social Worker will report directly to the Manager/and or Director of Clinical Operations. The Social Worker performs telephonic consults to identify and assess available services to meet the social service need of the member. The Social Worker assures that members and families are knowledgeable of potential community resources and how to access them. The Social Worker serves as a resource to clinical services staff in dealing with complex psychosocial issues and accessing community resources. You’ll enjoy the flexibility to work remotely from anywhere within the U.S. as you take on some tough challenges.

Requirements

  • Master’s Degree in Social Work (LSW)
  • Professional License in good standing for respective discipline
  • 2+ years professional experience in a social work position with adults in the Medicaid/Medicare population
  • Experience with arranging community resources
  • Knowledge of healthcare delivery system
  • Excellent written and verbal communication skills
  • Computer/typing proficiency to enter/retrieve data in electronic clinical records
  • Proficient with Microsoft Word, Excel, Outlook
  • Ability to work independently, as well as a member of a team
  • Solid interpersonal skills
  • Knowledge of medical terminology
  • Required to have a dedicated work area established that is separate from other living area and provides information privacy
  • Must live in a location that can receive UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

Nice To Haves

  • Telephonic member engagement experience
  • Knowledge of CMS regulations, Medicare and Medicaid
  • Bilingual a plus

Responsibilities

  • Performing telephonic social work consultations with members promoting high quality community resources
  • Performing telephonic consults with member’s PCP and/or physicians when needed
  • Ensure member has community resources needed
  • Documenting clinical consultations and resources given, into internal database
  • Maintaining the clinical integrity of programs offered
  • Serving as a clinical resource and consultant for other clinicians
  • Participating in and contributing to staff meetings and other staff development opportunities and interdepartmental work groups
  • Working with Manager to identify system improvements that could be made to drive operational advancements and efficiencies
  • Provide cross-coverage support across the team support and assist with special projects, as needed
  • Other duties as assigned by Manager or Director
