RN Care Coordinators lead and are accountable for providing exemplary cross continuum care coordination and care transitions, and are responsible for partnering with patients, families, physicians and colleagues to develop, implement and monitor a comprehensive plan of care; inclusive of health promotion, disease prevention, and disease management activities. Requirements

  • 3 years of recent clinical experience required, leadership experience preferred. If ADN, 10+ years of experience required.
  • Associates Degree OR BSN required upon hire; if Associates, BSN required within 3 years of hire
  • Master’s Degree in Clinical Discipline strongly preferred.
  • Active PA RN license required
