About The Position

Northside Hospital is an award-winning, state-of-the-art, and continually growing institution, constantly expanding the quality and reach of its care to patients and communities. This role involves working with neonate, adolescent, adult, and geriatric patients, families, medical staff, and community agencies to determine the appropriate setting of care and appropriateness of hospitalization, as well as to assess post-hospital care needs. The position also provides support to address social drivers of health.

Requirements

• Graduate of an accredited Registered Nursing School (RN)
• Current License in the State of Georgia as a Registered Nurse or Nursing Compact (NLC/eNLC) or Multistate License
• Two (2) years previous clinical experience.

Nice To Haves

• BSN
• Three (3) years of experience in case management, social services or related field.
• Certified Case Manager.
• Ability to interact with patients, families, physicians and hospital staff.
• Excellent oral and written communication skills.
• Ability to type accurately at an approximate rate of 30 wpm.
