About The Position
The Care Coordinator, RN is a Registered Nurse who serves as an advocate and shares a commitment to excellence, innovation and compassionate care for patients with advanced heart failure and pulmonary hypertension. The Care Coordinator, RN will collaborate with an interdisciplinary team to navigate the plan of care and facilitate the coordination of services across the health system. work well on a team and collaborate closely with patients and team members. The Care Coordinator, RN will navigate plan of care and coordination of services within the health system. Serves as an advocate for patients and their families. Helps with program development and growth. The Care Coordinator, RN uses critical thinking skills to assist with ongoing analysis and implementation of processes to ensure quality outcomes, program development, and growth. The Care Coordinator, RN exhibits critical thinking skills in planning and prioritizing nursing care.
