The Care Coordinator is responsible and accountable for coordinating care throughout the continuum of care for an assigned patient population. Care Coordination in the hospital and healthcare system is a collaborative practice model. In partnership with the patient, family, and other caregivers, the Care Coordinator will work with the multidisciplinary team, Providers, Nurses, Social Workers, financial counselors, and other ancillary staff to actively facilitate those functions associated with moving the patient through the continuum of care.

  • Responsible and accountable for prescribing, delegating and coordinating patient care.
  • Uses clinical judgment based on nursing skills to globally assess the patient's situation and develop an appropriate plan of care.
  • Accountable that patient care meets standards of safety, effectiveness, patient rights and guest relations.
  • Oversees care delivered by patient care team; coordinates plan of care.
  • Provides education and facilitates learning for patients, families, and patient care team.
  • Collaborates with physicians, families and other healthcare professionals to assist in developing and implementing an appropriate plan of care.
  • Advocates for the patient, represents the concerns of the patient/family and identifies and assists in resolving ethical and clinical concerns.
  • Delivers care with a team-orientation, emphasizing good customer relations and sound clinical judgment.
  • Completes and analyzes comprehensive assessment with patient intake.
  • Coordinates treatment plans including payors, supplies, medications, and community resources.
  • Ensures consults, testing and procedures are sequenced appropriately.
  • Utilizes disease-specific clinical pathways for effective clinical management.
  • Assesses educational needs of patients and families considering barriers to care.
  • Coaches patients/families toward lifestyle changes and successful self-management.
  • Facilitates communication and coordination of the plan of care with Providers and the healthcare team.
  • Identifies and addresses barriers in service or treatment.
  • Evaluates patient response to treatment regimens.
  • Works with multidisciplinary staff to ensure appropriate information is provided prior to transition.
  • Identifies and solves problems related to discharge needs.
  • Communicates and resolves conflicts with Providers and healthcare team members.
  • Builds therapeutic relationships with patients, families and caregivers.
  • Works to improve quality through reduction in treatment delays and monitoring of quality indicators.
  • Provides ongoing consultation and training to medical staff on discharge and home care issues.
  • Collects, completes and submits statistical data in a timely manner.
  • At least 4 years Pediatric nursing, Case Management, Care Management, Care Coordination, Utilization Review, or Community-based nursing required.
  • BSN required.
  • Registered Nurse in the State of Texas Upon Hire required.
  • Basic Life Support for Healthcare Providers as required by CP 1.20 Life Support Course Upon Hire required.
  • Accredited Case Manager (ACM) or Certified Case Manager (CCM) or Care Coordination and Transition Management (CCTM) Upon Hire preferred.
