About The Position

This position supports patient centered care acting as an advocate to patients and their families by navigating them through the health care system and removing barriers to health. The Care Coordinator works specifically with high risk patients experiencing complex disease processes needing multiple sources of health care support. The Care Coordinator provides clinically based holistic care coordination in the inpatient and outpatient setting by working closely with clinic/hospital/specialty providers and staff, and community resources in a team approach to reduce patient risk of readmission.

Requirements

  • Associate's Degree (AA) Graduate of an accredited school of nursing (ADN) Required.
  • Registered Nurse Iowa Board of Nursing (IBON) (United States of America) Upon Hire Required.
  • Child Abuse Iowa Mandatory Reporter - Iowa Department of Human Services within 180 Days Required.
  • Dependent Adult Abuse Mandatory Reporter - Iowa Department of Human Services within 180 Days Required.
  • 1-3 years Hospital/clinic experience Preferred.

Nice To Haves

  • Bachelor's Degree in Arts/Sciences (BA/BS) In Nursing (BSN) Preferred.
  • Basic Cardiac Life Support & First Aid - American Heart Association Preferred.

Responsibilities

  • Provides care coordination services for high risk and complex patient populations including but not limited to: Chronic Care Management, ED and Inpatient utilization reduction, Hospital discharge to home.
  • Promotes behavior change and develops care plans for patients.
  • Includes the use of Motivational Interviewing concepts.
  • Increases comprehension through culturally and linguistically appropriate education and the use of evidence-based approaches such as teach-back methodology.
  • Creates and promotes adherence to a care plan, developed in coordination with the patient, primary care provider, and family/caregiver(s).
  • Connects patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs.
  • Assesses and works on the patient's readiness to change, the importance of change and confidence in ability to change.
  • Assists all patients through the healthcare system by acting as a patient advocate and navigator.
  • Supports patient self-management of disease and behavior modification interventions.
  • Participates in quality improvement initiatives.
  • Maintains accurate and timely documentation and patient tracking.
