The Care Coordinator at Community Alliance plays a crucial role in supporting individuals with mental illness, particularly those reentering society after incarceration. This position involves utilizing effective communication skills, maintaining knowledge of community-based behavioral health services, and advocating for clients in the community. The Care Coordinator will work within an integrated health care approach, focusing on recovery and person-centered care.

  • Demonstrate and utilize effective oral and written communication skills.
  • Maintain and utilize current knowledge of community-based behavioral health services and integrated care.
  • Serve as an effective advocate for homeless persons in the community.
  • Complete all training as outlined in organizational policy.
  • Initiate a professional development and training plan and complete it annually.
  • Demonstrate and maintain functional computer skills, including electronic health record usage.
  • Earned Bachelor's degree in social work, rehabilitation, education, or relevant field from an accredited institution, or current certification as a peer support specialist in Nebraska.
  • Demonstrated knowledge and skills in mental health rehabilitation and recovery principles.
  • Knowledge of characteristics and needs of persons with serious mental illness, including co-morbid medical conditions.
  • Knowledge of available behavioral health services, social services, and basic case management principles.
  • Ability to maintain effective working relationships with diverse individuals and groups.
  • Experience with motivational interviewing and trauma-informed care.
  • Familiarity with the recovery model and health home model of integrated care.
