About The Position
Care Coordinators assist Spindletop Center clients to obtain support services needed to facilitate their recovery. Care Coordinators assist with gathering information necessary for the initial intake assessment and for the development of the Person Centered Recovery Plan. Care Coordinator assists the individual in obtaining needed laboratory, medical, social, financial and other services in support of their recovery. Care Coordinator follows-up on orders/referrals and problem solves as necessary to ensure services are obtained and to minimize the chances of the individual being lost to follow-up.
