About The Position

The Care Coordination Training Coordinator is an integral part of the Health Homes team. The role of the Care Coordination Training and Quality Coordinator is to provide ongoing training and support to newly hired and existing Care Managers and Patient Navigators in an effort to improve delivery of Health Homes care coordination services to enrolled Health Homes Patients. In addition, the Care Coordination Training and Quality Coordinator will serve as a point person for the Care Managers and Patient Navigators to receive support around removing barriers to the effective delivery of holistic high quality care coordination services while assessing gaps in performance and developing appropriate hands-on training activities to remedy such and increase performance targets

Requirements

  • BA/BS required
  • 2+ years experience in Care Coordination preferred

Nice To Haves

  • Certification in training or Train the Trainer a plus

Responsibilities

  • Prepares on boarding packages for all new Care Managers
  • Conducts orientation training for all newly hired Care Managers and Care Coordinating Patient Navigators
  • Develops and regularly updates Care Manager training manual
  • Maintains record of all trainings conducted
  • Provides ongoing support to Care Managers and Care Coordinating Patient Navigators in all care coordination inclusive of patient interactions, fieldwork activities and documentation of interventions performed
  • Conducts regular quality reviews to identify performance gaps, training opportunities and need for Quality Improvement projects
  • Works collaboratively with Care Coordinator around improving Care Manager and Care Coordination Patient Navigator productivity
  • Conducts ongoing and corrective training for assigned Care Managers and Care Coordinating Patient Navigators
  • Complete quality assessment activities to ensure and support adherence to established workflows and standards
  • Provides comprehensive care coordination services for a caseload of # 10 patients which will serve as a source for training and modeling of expected service delivery standards
  • Participate in QA and QI projects
  • Responsible for providing and training care coordination staff in;
  • Developing patient focused care plans with documented input and approval from providers and the patient in compliance with Health Homes standards
  • Providing Crisis intervention and health education services as needed
  • Working with the medical staff to develop, implement, and coordinate the care plan for patients with chronic diseases, mental illness and substance use problems based on the Health Homes chronic disease care coordination model standards
