We are seeking a Licensed Practical Nurse (LPN) to join our leadership team as a Care Coordinator . This is a field-based role focused on client assessments, caregiver supervision, and ensuring exceptional care from day one. This position is ideal for an LPN who wants more autonomy , variety , and purpose than a traditional clinical setting.

Requirements

• Active LPN license (required)
• Self-starter who works well independently

• Outstanding communication and people skills
• Strong organization and attention to detail
• Comfortable traveling locally (Canton, Ellijay, Dawsonville, Cartersville)
• Flexible availability
• Professional, calm, and solution-oriented

Responsibilities

• Conduct in-home assessments for new clients and create care plans
• Perform supervisory and follow-up visits with existing clients
• Supervise, coach, and support our caregiver team
• Lead caregiver introductions for new clients (30–60 minutes per introduction)
• Translate care plans into clear, actionable guidance for caregivers
• Use WellSky to review schedules and coordinate care
• Work with leadership and our RN to resolve client concerns
• Participate in weekly leadership team meetings
• Occasionally step in to cover caregiver shifts if needed
