Responsibilities Position Objective: The Care Coordinator Liaison is an administrative role, and is responsible for coordinating the delivery of care to patients, and ensuring that the care provided meets our value of 5-Star Service.

Requirements

  • Highschool degree or equivalent required
  • Prior experience working in a clinical setting, preferred
  • Prior experience working with electronic medical record systems, preferred
  • Strong telephonic skills to include communicating with patients, facilities, and other physician offices
  • Demonstrated ability to handle data with confidentiality
  • Ability to work cross-functionally with multiple teams; ability to work independently with minimal supervision
  • Excellent organizational, time-management, and multi-tasking skills with strong attention to detail
  • Excellent written and verbal communication skills; must be comfortable communicating with patients, vendors, and outside practices
  • Basic computer skills, including familiarity with electronic medical records
  • Strong critical thinking and problem-solving skills
  • Must be results-oriented with a focus on quality execution and delivery
  • Exceptional interpersonal and customer service skills
  • Working knowledge with Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaboration
  • Appreciation of cultural diversity and sensitivity toward target patient population
  • Full range of body motion, including handling and transferring patients, manual and finger dexterity; including eye-hand coordination
  • Requires standing, walking, pushing, bending, kneeling, and reaching at arm’s length and overhead in a clinic for prolonged periods of time
  • Requires ability to sit or stand for prolonged periods of time
  • Requires ability to lift/carry up to 15 lbs
  • Requires corrected vision and hearing to normal range
  • Ability to sit for extended periods of time
  • Ability to operate a computer and telephone

Responsibilities

  • Schedule patients to be seen at least 4 times per year, including scheduling patient follow-up visits and rescheduling canceled appointments and no-shows
  • Support Transition of care phone calls to include completion of medical records requests post-discharge (INP and ER)
  • Schedule timely patient post-discharge follow-up appointments (INP and ER)
  • Schedule new patients to be seen by their PCP within 30 days of their effective date
  • Obtain prior patient records and ability to use new patient registration process for scheduling
  • Facilitate HEDIS/Stars call outreach to include Part C and Part D Gap Reports
  • Facilitate patient referral coordination to include: Concierge Referral Process to assist patient scheduling with a specialist
  • Obtain records from specialist post-visit and upload documents into the patient chart/EMR for PCP review
  • Telephone outreach to disenrolled members
  • Assist with medical records transfers
  • Connect patients with resources within the community to improve patient outcomes
  • Additional duties as assigned
