North Texas Behavioral Health Authority (NTBHA), based in Dallas is responsible for developing a collaborative system of care for children, adolescents and adults with severe mental illness and other serious emotional disturbances, as well as those experiencing a substance use disorder. The Care Coordinator at the Kaufman Livingroom provides timely intervention in the least restrictive level of care; that is comprehensive, trauma-informed, person-centered, recovery and resilience–oriented efforts, to help people in crisis gain recovery and stability in the community. Their role will primarily serve Kaufman County and requires excellent organizational and clinical skills to provide continuity of care and crisis response solutions through intersystem and interdepartmental functions, and processes through the NTBHA Utilization Management Department and community stakeholders. The Care Coordinator provides alternatives to arrest, emergency department utilization and inpatient psychiatric care. Disposition planning consists of performing complex services that includes screenings and assessments, crisis follow up and transitional services that effectively connect individuals with mental illness and/or a co-occurring disorder to other services preventing re-emerging crisis, while eliminating barriers to treatment. The Care Coordinator actively participates in staffing's to determine best treatment options, collaborate and work effectively with internal departments, law enforcement, state and local hospitals, and community agencies and organizations. This position will provide skills training and case management targeting identified individual risks and needs relative to reducing recidivism, hospitalization and/or homelessness. The Kaufman Livingroom Center Coordinator requires minimal travel with an expectation of reliable transportation and valid driver’s license and car insurance required. May require cellular availability after hours and flexible work hours that include evenings and possibly one day weekend every other week and one, 12-hour shift per month to meet program needs. Travel is required as part of this role’s duties. Travel can vary from 20% to 100% per day. This would include all counties within NTBHA Catchment area (Dallas, Ellis, Hunt, Kaufman, Navarro & Rockwall Counties). This role may be required to use a fleet vehicle and and/or a personal vehicle for business needs. This may include, but not limited to field & home visits, offsite meetings, co-located sites and/or pick up items for business needs, etc. If your program requires transport of individuals, this would only be allowed with a company vehicle and approval from management.
