About The Position
We are looking to hire a Care Coordinator to provide care coordination services to individuals enrolled in our One Care program-a program for patients with Medicaid and Medicare coverage between the ages of 21-64 years of age. You will work on a small, collaborative team with One Care Nurse Care Managers and other Care Coordinators as well as our medical teams to ensure that patients are being linked to needed services to improve their health and overall well-being and meeting goals outlined on their care plan. In this role, you will also be responsible for spending approximately 20% of his or her time supporting administrative functions for the One Care team. A significant aspect of this work; however, will be direct patient engagement that will be a combination of telephonic and face-to-face outreach.
