Job Summary The Care Coordinator, Geriatrics will provide support to the nursing staff, medical providers, and care management focused on delirium prevention in the elderly inpatient care. Reports to: Job Specific Responsibilities Daily assignments may include but are not limited to: • Patient screening for delirium • Coordinate volunteers and Elder Life Advanced Practice Provider care • Coordinates care to prevent delirium onset in the elderly inpatient population. • Facilitates care management in collaboration with the Program Director, Geriatrician, Primary Provider, and Advanced Practice Provider for geriatric patients at risk for delirium. • Ensures inpatient-outpatient transition and continuity of care on an on-going basis. • Identifies, implements, and reports quality improvement initiatives pertaining to patient safety, patient care and care efficiency. • Provide metrics, stats, reporting, and new goals related to delirium progressions and opportunities • Serves on the Delirium Taskforce. • Organize and implement Daily Safety Huddle to review next day’s patient list and therapeutic interventions. • Serves as a liaison with other health care specialties in the hospital and with community agencies to facilitate optimal inpatient care and referrals after discharge. • All other assigned duties related to delirium and/or elder care

Requirements

  • Bachelor’s Degree in Science, Human Services, or related Healthcare field
  • + 1 year of experience in geriatrics or senior clinical care
  • + 2 years of experience successfully leading people and/or processes
  • BLS: Basic Life Skills certification by The American Heart Association
  • Able to champion national standards of care for the Geriatric population, specific to delirium prevention.
  • Clear and concise communication skills, both verbally and in writing
  • Proficient in Microsoft Office, including proficiency in spreadsheets and data compilation
