Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Reston, Virginia area, with a primary focus on the Fairfax vicinity. This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support. WHAT 'EMBEDDED' MEANS AT ENNOBLE CARE This is not a remote or call-center role. As an Embedded Care Coordinator, you are physically on-site in one or more partnered long-term care facilities, working alongside providers and facility staff. You build consistent relationships with residents and families and become a trusted, familiar presence within the facility -- supporting care the way a traditional family practice would, but in the long-term care setting.

Requirements

  • Legally authorized to work in the United States of America
  • Active, unrestricted clinical certification or licensure, such as: Patient Care Technician (PCT) Certified Nursing Assistant (CNA) Medical Assistant (MA) / Certified Medical Assistant (CMA) Licensed Practical Nurse (LPN) Registered Nurse (RN) Or comparable credential
  • Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
  • Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person
  • Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting
  • Well-developed organizational and time-management skills, paired with an optimistic approach and initiative-driven mindset
  • Strong working knowledge of healthcare terminology and care coordination principles
  • Proficiency with Microsoft 365, including Outlook, Word, and Excel
  • Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, in an on-site role, based in the Reston, Virginia area, with a primary focus on the Fairfax vicinity
  • No weekends
  • No on-call
  • Daytime hours
  • Seven (7) company-paid holidays annually
  • Transportation criteria: Valid, unencumbered driver’s license Reliable personally-owned vehicle Current auto insurance, with the ability to provide proof

Responsibilities

  • Coordinate care for long-term care residents using Ennoble Care’s EMR
  • Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions
  • Collaborate closely with Ennoble Care providers, within the long-term care setting
  • Serve as a key liaison between providers, facility staff, residents, and families
  • Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner
  • Accurately document patient interactions and care coordination activities
  • Follow established workflows, documentation standards, and escalation protocols
  • Contribute positively to team collaboration and strong facility partnerships
