About The Position

The Care Coordinator will conduct a needs assessment with the client and their family as requested, and provide intervention, consultation, and coordination of care across the spectrum of health services. The Care Coordinator will complete screening and assessments, provide person and family-centered treatment planning for adults and children, to include health promotion, individual and family support, and referrals to CCBHC services and community agencies. The Care Coordinator will assist with discharge planning from inpatient and residential MH and SUD services to community-based care.

Requirements

  • Requires bachelor’s degree with at least 30 semester hours or equivalent quarter hours in a human services field, including but not limited to sociology, social work, counseling, criminal justice, psychology, or human services.
  • All required state certifications and/or licensure as dictated by employee’s academic degree.
  • All state, federal, or proprietary body certifications as required to hold a specified position or perform a specific task/service i.e. IM CANS, etc.
  • Experience working with children and adolescents with serious emotional disturbance, and adults with severe and persistent mental illness.

Responsibilities

  • Assures that all clients receive clinically sound triage/referral and ongoing care management services for behavioral health needs.
  • Coordinates treatment plans and follows clients through the treatment process.
  • Requests consultation for clients that includes providers, social workers and nursing staff when necessary.
  • Delivers high quality care appropriate to the age, developmental level, and educational level of the client.
  • Triages phone calls to distinguish between emergent, urgent, and routine calls.
  • Responds to phone calls for crises intervention by providing brief direction, recommendations, and crises intervention.
  • Assists clients in enrollment of state benefits, and connection to additional resources to meet needs related to Social Determinants of Health (SDOH).
  • Review medications prescribed, and with client consent, provide prescription information to other providers to prevent opioid misuse.
