About The Position

The Care Coordinator/Case Management position involves providing targeted mental health case management for youth with serious emotional disturbances and co-occurring disorders. The role includes meeting with enrolled and potential clients and their families to create a Plan of Care, conducting comprehensive assessments as required by DHMH, and coordinating Family Team Meetings. The Care Coordinator will manage the Plans of Care (POCs), collect necessary information during the application process, and identify providers, supports, and resources for clients. Additionally, the position requires coordinating meetings with clients and families to ensure the appropriateness of proposed services, maintaining client clinical documentation using EHR, providing on-call services, responding promptly to client crises, and facilitating groups.

Requirements

  • Bachelor's degree in a mental health field and 1 year of mental health experience, including mental health peer support; or Bachelor's degree in a field other than mental health and 2 years of mental health experience, including mental health peer support
  • Ability to handle stressful situations
  • Must maintain a flexible work schedule to meet scheduling needs of clients
  • Must meet Care Coordinator training and certification requirements
  • Must complete the training required by CSA and BHA, including CASII, ESCII, and CANS
  • Must complete Wraparound certification.

Responsibilities
