About The Position

The Care Coordinator at Esperanza Health Centers is responsible for providing care management to a group of patients, assisting in prevention efforts to help them achieve and maintain optimal health. This hybrid role requires a flexible schedule, including evenings and weekends, and involves working both onsite and offsite. The Care Coordinator acts as a resource for patients and healthcare staff, ensuring that patients receive the necessary care and services in a fast-paced clinic environment.

Requirements

  • Associate's degree or equivalent work experience required.
  • Three or more years of clinical experience required.
  • Experience as a Medical Assistant, Case Manager, or Health Educator preferred.
  • Previous experience with Electronic Health Records preferred.
  • Knowledge of Microsoft Office (Excel, Word) preferred.
  • Bilingual in English and Spanish required.

Responsibilities

  • Communicate with the care team and review patient charts to identify gaps in care.
  • Schedule appointments and participate in the establishment of care plans.
  • Facilitate communication among patients, providers, nurses, and medical assistants.
  • Maintain timely communication with patients regarding their healthcare and resource needs.
  • Refer patients to community resources and ensure services are received.
  • Help patients connect to primary care, specialty, and ancillary services by providing information and making appointments.
  • Encourage patient engagement in their health through compassionate communication and motivational interviewing techniques.
  • Engage in community outreach with local schools, churches, and business partners.
