About The Position

The Care Coordinator - Behavioral position is a full-time role focused on providing recovery-oriented in-home and community-based services for adults with serious mental illness and children with serious emotional disturbances. The role involves comprehensive care coordination, health promotion, and support for individuals and families, ensuring access to necessary services and resources.

Requirements

  • Bachelor's degree in behavioral healthcare services or a related field.
  • Valid driver's license for work-related driving.

Responsibilities

  • Conduct outreach activities to engage members in care coordination.
  • Perform individualized, comprehensive assessments of members.
  • Schedule appointments and make referrals to appropriate services.
  • Track referrals, appointments, and HEDIS measures.
  • Monitor follow-up communications with providers regarding member goals.
  • Support coordination of care with primary care providers and specialists.
  • Promote members' health and include personal health goals in treatment plans.
  • Engage members and/or caretakers to provide alternatives to emergency care.
  • Participate in the hospital discharge process and perform medication reconciliation.
  • Facilitate the development of crisis plans and monitor for potential crises.
  • Conduct follow-up phone calls and face-to-face visits with members and families after discharge.
