About The Position
Abbe Center for Community Mental Health is seeking a full time Care Coordinator for our Integrated Health Home (IHH) program in Independence office. Care Coordinators work with a team of professionals to enhance the physical and mental well-being of clients in the IHH program. This position includes Integrated Health Home hours that are office-based and work in client's homes in the Independence area. Monday-Friday daytime hours with possibility of a limited work-from-home schedule and flexibility in hours.
