The Care Coordinator works as a core member of the Collaborative Care Team that involves the patient’s primary care provider, nurse, medical assistants, and/or mental health and substance use disorder (SUD) providers in the health center and through community partnerships. The Care Coordinator is responsible for coordinating and supporting primary medical, dental, behavioral health, and SUD care within the health center, as well as linking patients with external services and recovery supports. This position plays a key role in supporting Alluvion Health’s Substance Use Disorder programs, including coordination with Veterans Treatment Court, Adult Treatment Court, and Jail/Prison Community Reentry Programs. The Care Coordinator assists with referrals, patient navigation, treatment adherence, data entry, and communication between courts, probation officers, and SUD providers. The Care Coordinator provides excellent customer service to patients and providers while ensuring timely access to care, assessing for additional supportive services, and facilitating whole-person care through care coordination and system navigation. The Care Coordinator is or will be certified as a SHIP Counselor and Certified Application Counselor to provide outreach and enrollment services for Medicare, Marketplace insurance and/or Medicaid enrollment and participates in clinical huddles, care planning, and community outreach activities as assigned.
