About The Position

The Care Coordinator works as a core member of the Collaborative Care Team that involves the patient’s primary care provider, nurse, medical assistants, and/or mental health and substance use disorder (SUD) providers in the health center and through community partnerships. The Care Coordinator is responsible for coordinating and supporting primary medical, dental, behavioral health, and SUD care within the health center, as well as linking patients with external services and recovery supports. This position plays a key role in supporting Alluvion Health’s Substance Use Disorder programs, including coordination with Veterans Treatment Court, Adult Treatment Court, and Jail/Prison Community Reentry Programs. The Care Coordinator assists with referrals, patient navigation, treatment adherence, data entry, and communication between courts, probation officers, and SUD providers. The Care Coordinator provides excellent customer service to patients and providers while ensuring timely access to care, assessing for additional supportive services, and facilitating whole-person care through care coordination and system navigation. The Care Coordinator is or will be certified as a SHIP Counselor and Certified Application Counselor to provide outreach and enrollment services for Medicare, Marketplace insurance and/or Medicaid enrollment and participates in clinical huddles, care planning, and community outreach activities as assigned.

Requirements

  • Associate or bachelor’s degree preferred.
  • 3–5 years of related experience in SUD treatment, case management, or court-based programs preferred.
  • Experience in a Federally Qualified Health Center (FQHC) or healthcare setting a plus.
  • Certification as a Community Health Worker, Behavioral Health Aide, Medical Assistant, or LPN preferred.
  • Certified Application Counselor or SHIP certification preferred (or willingness to obtain).
  • Valid Montana driver’s license required.

Responsibilities

  • Serve as liaison between Alluvion Health and Veterans/Adult Treatment Courts and Jail/Prison Reentry Programs.
  • Attend multidisciplinary team and treatment court meetings to share progress updates and coordinate care plans.
  • Collaborate with detention centers, probation, and community partners to facilitate smooth reentry transitions and access to services.
  • Provide ongoing support, check-ins, and follow-up for individuals in court or reentry programs.
  • Track attendance, engagement, and compliance with treatment or court requirements.
  • Ensure timely access to behavioral health and SUD services.
  • Prepare and submit documentation for courts, probation officers, and case managers (attendance logs, compliance updates, etc.).
  • Maintain accurate and timely records in electronic databases (DIMS, EMR).
  • Assist with data tracking, outcome reporting, and program evaluation.
  • Coordinate care between SUD providers, primary care teams, and external partners.
  • Support pre-visit preparation, chart updates, and provider communications.
  • Identify and remove barriers to patient care, including insurance or transportation challenges.
  • Represent Alluvion Health’s SUD and reentry programs at meetings and community events.
  • Participate in outreach, harm reduction, and education initiatives.
  • Maintain SHIP Counselor and/or Certified Application Counselor certification to assist with patient insurance navigation.
