About The Position

The Care Coordinator position at Riverside Community Care involves providing person-centered and recovery-oriented care management and coordination for eligible MassHealth consumers in the community. The role focuses on outreach, engaging consumers in services, identifying care goals, and facilitating access to community resources, all while working within a multi-disciplinary team. This position offers a unique opportunity to gain experience in community outreach and support individuals with medical and behavioral health needs.

Requirements

  • Bachelor's degree in a behavioral health or social service field.
  • LADC II Licensure.
  • Associate's degree in a behavioral health or social service field with one year of professional or lived experience.
  • High School Diploma/GED with two years of professional or lived experience.
  • Experience working with substance use disorders preferred.
  • Experience with MS Office, Electronic Health Records, and email strongly preferred.
  • Valid driver's license and regular access to reliable transportation required.

Responsibilities

  • Provide person-centered and recovery-oriented care management and coordination to eligible MassHealth consumers.
  • Engage eligible consumers in services through outreach efforts.
  • Assist consumers in identifying their individualized care goals.
  • Identify appropriate community-based resources for consumers.
  • Communicate effectively with the multi-disciplinary team.
  • Complete ongoing needs assessments for consumers.
