About The Position

Quality Care seeks a full-time Care Coordinator Intern. and direct support person, working 70% as a DSP and 30% as a Care Coordinator assistant to join our team. Care Coordinator Assistant – Gain hands-on experience in care management, program coordination, and support services for individuals and their families. Direct service provider: As a caregiver, you will provide In-home services (IHS) to individuals living in their residences with varying mental and physical health needs. This position is an excellent opportunity for individuals with flexible weekday schedules wanting to connect with people. Requirements

  • Strong interpersonal skills, patience, and empathy.
  • Associate degree or higher in human services, social work, or related field preferred.
  • Previous experience working with individuals with developmental or intellectual disabilities, mental health challenges, or in a caregiving role preferred.
  • Strong organizational, verbal and written communication, and problem-solving skills.
  • Ability to provide compassionate, person-centered care while promoting independence.
  • Capable of following individualized care plans, instructions, and safety protocols.
  • Basic computer literacy for documentation and reporting.
  • Ability to manage multiple tasks and respond calmly in challenging situations.
  • Valid driver’s license and reliable transportation (Required).
  • Pass background checks.

Responsibilities

  • Assist with care planning, coordination of services, and client assessments.
  • Support communication between clients, families, and multidisciplinary teams.
  • Participate in meetings and contribute to program development initiatives.
  • Document and maintain accurate records of care plans and client progress.
  • Report office progress to the Lead Care Coordinator and work with them to improve office operations and procedures
  • Companionship, interpersonal connection, and emotional support
  • Provide compassionate assistance with daily living activities, including meal preparation, laundry, light housekeeping, errands, shopping, and recreational support.
  • Transportation to medical appointments.
  • Implement individualized care plans while promoting independence and community integration.
  • Ensure the safety and well-being of individuals in home and community settings.
  • Maintain accurate documentation and communicate effectively with team members and supervisors.
