About The Position

The Care Coordinator for Adult Outpatient at Eskenazi Health plays a crucial role in providing skills training and development to clients with mental health concerns. Requirements

  • Bachelor's degree required.
  • If holding a bachelor's of social work degree, must possess a current, unencumbered Licensed Bachelor's Social Worker license in Indiana.
  • Valid Indiana driver's license and a driving record that meets Eskenazi Health's driving policy requirements.

Nice To Haves

  • Basic knowledge of behavioral health.
  • Basic knowledge of substance use disorder and/or co-occurring disorders.
  • Basic knowledge of medical terminology.

Responsibilities

  • Provide skills training and development to clients in group and individual settings as guided by medical necessity.
  • Conduct necessary client assessments and revise treatment plans in collaboration with the treatment team.
  • Provide crisis management and emergency interventions as needed.
  • Report to the treatment team on client progress, challenges, and any changes in level of care.
  • Ensure timely completion of all documentation and billing according to guidelines.
  • Establish and maintain a safe environment through ongoing assessment and intervention.
  • Provide case management by working with family and community supports to assist clients in meeting their goals.
  • May provide transportation in a company vehicle or personal vehicle as directed.
