About The Position

The Care Coordinator, Social Worker II at Orlando Health Winnie Palmer Hospital for Women & Babies is responsible for collaborating with clinical teams to provide care coordination services. Requirements

  • Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
  • One year of direct clinical experience with an emphasis on the population served or a completed internship in healthcare.

Responsibilities

  • Lead the continuity and consistency of care across inpatient, emergency, and outpatient settings.
  • Develop effective working relationships with the Care Management Team to engage patients and families in their care.
  • Monitor progress towards discharge plans and adjust as necessary based on patient condition or family needs.
  • Educate patients and families about evidence-based standards of care and self-management strategies.
  • Identify support needs for patients and families, developing action plans to address them.
  • Facilitate access to prescriptions, durable medical equipment, and other necessary services.
  • Contribute to team problem-solving through communication, collaboration, and data collection.
  • Advocate for patients to optimize their healthcare needs, including safety and financial well-being.
  • Refer patients to educational resources and community services as appropriate.
  • Utilize IT resources to improve disease outcome measures and maintain patient populations.
  • Participate in clinical outcome measurement and promote population health strategies.
