About The Position

The Care Coordinator for Acute services at Orlando Health is responsible for collaborating with the assigned clinical team to identify patients who would benefit from care coordination services. This includes assessing patients' risk factors and the need for clinical utilization management and preventative care services. The role emphasizes ensuring continuity and consistency of care across various settings, including inpatient, emergency, and outpatient care. The Care Coordinator will facilitate comprehensive discharge planning and follow-up care, develop effective working relationships with Patient and Family Counselors/Social Workers, and monitor patient progress towards discharge plans.

Requirements

  • Master's degree from an accredited school of Social Work, Mental Health, Psychology, or Marriage and Family Therapy.
  • Two (2) years of direct clinical experience with an emphasis on the population to be served.
  • Successful completion of a Master's level internship within the population to be served may substitute for the two (2) years of experience.
  • Handle with Care (HWC) Certification required for Behavioral Health Unit.

Responsibilities

  • Lead the continuity and consistency of care across inpatient, emergency, and outpatient settings.
  • Facilitate comprehensive discharge planning and follow-up care.
  • Develop effective working relationships with Patient and Family Counselors/Social Workers and UR nurses.
  • Monitor progress towards discharge plans and adjust as necessary based on patient condition or family needs.
  • Educate patients and families with chronic illnesses about evidence-based standards of care and self-management strategies.
  • Identify support needs for patients and families, develop action plans, and provide guidance for self-management.
  • Educate patients and families about the healthcare system and facilitate relationship building across various settings.
  • Ensure patients have access to prescriptions, durable medical equipment, and other identified services.
  • Contribute to team problem solving through communication, collaboration, and data collection.
  • Advocate for patients to optimize their healthcare needs, including safety, physical, legal, and financial well-being.

Benefits

  • Flat rate of $33.00 per hour plus differentials.
  • Flexible scheduling with a requirement to work 4 shifts in a 6-week period to maintain a pool position.
