Requirements

  • Bachelor's degree in social work, psychology, or related field from a regionally accredited institution. In some cases, a waiver may be granted by the Illinois Department on Aging if employee has significant case management or significant experience in aging services.
  • Minimum of three months of experience in a related field.
  • Fundamental communication and problem-solving skills.
  • Ability to work independently and respond appropriately in crises.
  • Relate well to individuals with disabilities and older adults, respecting their privacy and dignity.
  • Effective and constructive interaction with diverse cultures and backgrounds.
  • Proficiency in Microsoft Office applications (Outlook, Word, Teams).
  • Strong verbal and written communication skills.
  • Valid driver's license, reliable transportation, and proof of liability insurance.

  • Preferred experience with program-specific populations.

Responsibilities

  • Provide intake, screening, assessment, case planning, crisis intervention, and case management services.
  • Determine eligibility for Illinois Department on Aging Waiver Program (Community Care Program).
  • Conduct comprehensive needs assessments and determine eligibility for services.
  • Establish written care or treatment plans with clients and families for maximum independence.
  • Active intervention and advocacy to access necessary services from various programs.
  • Complete monitoring, follow-up, and proper termination of cases.
  • Ensure the completion of Intensive Casework with each completed comprehensive assessment.
  • Complete critical incident reports in accordance with Illinois Department on Aging Standards.
  • Maintain confidential case records and meet documentation standards.
  • Accurate completion of all required reporting and documentation.
  • Maintain cooperative relationships with service providers and community organizations.
  • Conduct educational presentations for professional and community groups.
  • Attend training, staff, and team meetings, and act as an Agency ambassador in the community.
  • Other duties as assigned within the guidelines of this position.
