This specific role involves providing telephonic clinical assessment, nursing triage, health information, and patient education to a diverse patient population, including those recently discharged from in-patient admission. The Care Coordination Nurse I will direct patients to appropriate levels of care and collaborate with team members to ensure coordinated care.

Requirements

  • Must have excellent verbal and written communication skills with patients and all members of the healthcare team
  • Must have excellent organizational skills and ability to complete competing priorities
  • Must have advanced computer skills
  • Must be team oriented
  • Ability to use critical thinking skills and respond quickly and professionally under stressful, sometimes emergent situations
  • High School Diploma
  • Nursing-LPN field of study
  • LPN bedside nursing experience
  • LPN License
  • Multistate License issued under the NLC (Nurse Licensure Compact)
  • Must reside in Louisiana and able to work Eastern time zone (shift starts at 8:30am EST)

Nice To Haves

  • Home Health, Med Surg, or Emergency Department experience preferred

Responsibilities

  • Provide telephonic clinical assessment, nursing triage, health information and patient education to a wide variety of patients across the health continuum and/or health education to culturally diverse population recently discharged from in-patient admission
  • Assists in directing patients to the most appropriate level of care
  • Work closely with other members of the team to ensure adequate coordination
  • Adheres to and participates in the Care Management process
  • Provide outbound follow-up calls as necessary
  • All other duties as assigned
