Position Overview The Care Coordination Manager plays a central, client‑facing role serving as the primary liaison between clients, families, caregivers, and internal teams. From the first phone call to ongoing support, you’ll lead the care coordination journey with warmth, professionalism, and meticulous attention to detail. Requirements

  • HHA, CNA, or LPN certification preferred.
  • Minimum of two years of home care or healthcare experience.
  • Experience with home care staffing and scheduling strongly preferred.
  • Strong communication, interpersonal, and organizational skills.
  • Ability to work independently, manage multiple priorities, and thrive in a fast‑paced environment.
  • Comfortable learning and using scheduling and client‑management systems.
  • A passion for advocating for seniors and providing compassionate, high‑quality care.

Responsibilities

  • Serve as the primary point of contact for client inquiries and referrals, offering prompt, professional, and compassionate support.
  • Lead the intake and discovery process to understand client needs, preferences, and goals.
  • Guide clients and families through the transition from inquiry to active home care services.
  • Coordinate admissions and collaborate closely with nursing, scheduling, and operations teams.
  • Maintain strong, ongoing communication with clients and families—responding to questions, addressing concerns, and managing service updates.
  • Partner with scheduling coordinators to match clients with caregivers who support long‑term success and satisfaction.
  • Support onboarding and engagement for start‑of‑care teams.
  • Participate in community outreach and help build relationships with external care partners and referral sources.
  • Deliver exceptional customer service and act as a positive ambassador for Osborn Home Care in every interaction.
