About The Position
The Care Coordination Lead for Transitions of Care at AltaMed is a pivotal role that focuses on enhancing the quality of care for members during critical transitions. This position is responsible for gathering relevant information for the identified member population during assessment, care planning, interdisciplinary care team meetings, and transitions of care. The Care Coordination Lead will perform troubleshooting when problems arise and take independent action to resolve complex issues, ensuring that members receive the support they need throughout their care journey. This role is essential in bridging gaps in care and facilitating effective communication among healthcare providers, patients, and their families. The ideal candidate will be passionate about helping others and committed to making a meaningful difference in the lives of those they serve. In addition to direct care coordination, the Care Coordination Lead will collaborate with various stakeholders, including healthcare providers, social workers, and community organizations, to ensure a holistic approach to patient care. This position requires a deep understanding of healthcare regulations, particularly those governing Medi-Cal, Medicare, and other government programs. The Care Coordination Lead will also play a key role in identifying and addressing barriers to care, advocating for patients, and promoting health equity within the community.
