Each year, HCAM assists over 80,000 individuals across the state, offering personalized guidance, health education, and advocacy to ensure every Marylander has an equal opportunity to achieve optimal health. healthcareaccessmaryland.org Job Summary The Care Coordination Associate will assist Baltimore City Department of Social Services (BCDSS) Out of Home Placement (OHP) staff and foster parents with coordinating health care services for children in out of home placement, This coordination includes periodic case reviews of children assigned to a caseload and the scheduling of appointments for routine medical, dental, mental health and specialty care as needed.

Requirements

  • Bachelor’s degree in a Human Services field.
  • Experience in coordinating care and services.
  • Strong computer literacy skills, specifically with MS Word, Excel, Outlook and SharePoint (365) and Google Suite.
  • Organizational skills with the ability to meet a demanding workload
  • Detail oriented to complete requirements of files and contract compliance.
  • Sensitivity to cultural and socioeconomic characteristics of population served.
  • Excellent oral and written interpersonal and communication skills required
  • Problem solving skills
  • Proven ability to work efficiently and meet deadlines

Responsibilities

  • Administration Weekly attend Case Transfer meetings
  • Attend all required meetings assigned by HCAM and funders.
  • Serve as a resources person for BCDSS permanency staff for navigating the medical, dental and mental health systems.
  • Review monthly reports and resolve discrepancies.
  • Case Management Monthly review around 8-9 cases.
  • Close all assigned cases within 30 days.
  • Identify health care resources and make referrals to appropriate agencies
  • Schedule medical appointments for children under caseload
  • Track health care services for children in out-of-home placements to ensure their health needs are met.
  • Documentation Request all necessary documentation from medical providers
  • Document assessments, activities treatment plans, and resolutions .
  • Database Management Audit cases in database and ensure accuracy
  • Maintain and update medical information in the HCAM and CJAMS databases
  • Human Resources Timely and accurately submit timesheets.
  • Complete all required HCAM and funder training as assigned .
