About The Position

Position Summary: The Care Coordination Associate will function as an extension of the provider to help close the gaps in the patient's care for patients with complex medical needs; ensuring continuity of care and disease management for the patient. The care associate will build a rapport with patients through monthly calls to empower patients to meet their goals. This role is intermittent hybrid with remote functions and 2-5 onsite shifts for Bronx-based work monthly (more as needed).

Requirements

  • Highschool/GED Required.
  • 1-2 years of direct experience working with patients in a healthcare setting.
  • Experience and proficiency working with electronic medical records.
  • Ability to manage and support patient's healthcare needs, and complete individualized patient care plans along with performing care coordination.
  • Excellent interpersonal skills.
  • Computer skills, including ability to draft and create professional flyers.
  • Bilingual Spanish/English is required.

Nice To Haves

  • Associates degree in related area of study preferred.
  • Medical Assistant certification from an accredited program is preferred.

Responsibilities

  • Provides telephone support to patients which includes pre-visit planning, scheduling appointments, etc. etc.
  • Develops an NCQA CCMR care plan with pts/family that includes self management goals, barriers, needs, interventions.
  • Provides patient with referrals to health care team members including nurse care manager, SW, Health Educator, Nutritionist.
  • Assist patients with management of care transitions.
  • Adheres to documentation protocols and best practices for daily workflow, escalation of patient issues, and care plans.
