About The Position
This position provides violence intervention services to at-risk individuals in the Linden community. Promotes community resilience wrap-around services and linkages to community services (housing, employment, and social determinants of health). The Care Coordination Assistant builds a strong relationship with people in the neighborhood who are known to be or likely to be involved in gun violence to learn about and mediate issues and conflicts with the goal of using preventative measures to increase community resiliency and decrease acts of violence between residents and community members. Build positive relationships with residents in efforts to teach alternative ways to deal with conflict. Works 40 hours per week. The ability to work some evenings and weekends is expected.
