About The Position

The Care Coordination Assistant is responsible for assisting in providing a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services. This includes implementing care gap closure, supporting the provider to offer evidence-based medicine, managing the disease registry, and coordinating care activities across the care continuum. The role supports the mission and goals of Holy Cross Health through a patient-centered care approach, working collaboratively with provider care teams, Care Transformation colleagues, and other healthcare professionals to ensure quality and safe care for all patients within Holy Cross Health's CTO network of providers.

Requirements

  • Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA) required.
  • High school diploma required; Bachelor's Degree preferred.
  • Five years' experience in a healthcare setting, preferably in an ambulatory setting.
  • Moderate to high computer literacy with willingness to learn.
  • Ability to document/access effectively within multiple electronic health records.
  • Knowledge of Microsoft Office suite software required.
  • Knowledge of performance improvement, billing, coding, and documentation requirements highly desired.
  • Knowledge of evidence-based behavioral change methodologies and motivational coaching techniques highly desired.
  • Demonstrated ability to function effectively in a dynamic, fast-paced environment.
  • Customer-focused interpersonal skills to interact effectively with diverse individuals.

Responsibilities

  • Oversees the disease registry database.
  • Obtains, reviews, and screens patient lists for prioritization of outreach and necessity of clinical programs.
  • Assists patients and/or caregivers in scheduling appointments and accessing services.
  • Communicates and coordinates with healthcare teams to develop tools for optimal patient outcomes.
  • Facilitates care team meetings, prepares reports, and participates in follow-up.
  • Works collaboratively with providers and care teams to promote continuous quality improvement.
  • Supports patients and caregivers in self-management and oversees care coordination.
  • Creates plans for health behavior change for low-risk and high-risk patients.
  • Elicits patient goals and designs individualized treatment and self-care plans.
  • Provides or arranges needed patient education.
  • Acts as a patient's advocate and resolves patient concerns.
  • Liaises with patients and families to provider care teams and community services.
  • Consults with referring sources or provider care teams to review plans of care.
